I balmiki sharma,lives in jamshedpur jharkhand, my wife nitu kumari admitted on tmh hospital dated on 27/12/2021in emergency due to labour pain,and discharge 31/12/2021 patient no-ip/21/057097 i am job in railway,for medical reimbursements calim in railway want to emergency cft in railway format,but hospital denied to given emergency cft and sign in to railway page.that'swhy 75256 rs bill is hold.plz given to emergency cft in railway format, other hospital will do sign and stamp,plz see this matter, thank you
मै बाल्मीकि शर्मा जमशेदपुर झारखण्ड का रहने वाला हू,अपनी पत्नी नीतू कुमारी को दिंनाक 27/12/2021को इमरजेंसी मे जमशेदपुर के टाटा मुख्य असप्ताल मे तेज प्रसव पीड़ा होने पर भर्ती करवाया था,जिसमे 28december को ओप्रेसन से लड़की का जनम हुआ,तथा 31/12/2021को दिसचार्ज हूई, पेशेंट no--( IP/21/057097),मैं रेलवे मे जॉब करता हूँ,जहा मुझे मैडिकल reimbursment के लिये इमरजेंसी सर्टीफिकेट की जरुरत हैं रेलवे द्वारा एक इमरजेंसी फॉर्म दिया ग्या है,जिसमे हॉस्पीटल और डॉक्टर का साइन और मुहर होना चाईये,जो की हॉस्पिटल न्ही दे रहा,वो अपने पैड पर इमरजेंसी लिख कर दिये हैं लेकिन रेलवे वेरिफाय के लिये अपने फॉर्म पर साइन और मुहर चाहता है,जो की टाटा मुख्य अस्पताल न्ही कर रहा,महाशय इसके कारण 75256रूपये का बिल अटका हुआ हैं,कृपया कर टाटा मुख्य अस्पताल को इमरजेंसी cft देने के लिय कहे,क्या टाटा अस्पताल मुझसे ज्यादा बिल लिया है,जिसके कारण इमरजेंसीcft पर साइन करने से डर रहा,मैने सेवा के बद्ले मे पुरा बिल अदा किया हूँ फिर भी cft पर साइन देने से इन्कार कर रहे, कृपा कर मेरी मदद करे,इसके लिय मै आपका सदा आभारी रहूँगा,इमरजेंसी cft जिस पर साइन की जरुरत है,उसकी कॉपी सलन्ग हैं दुसरे हॉस्पिटल उस पेपर पर साइन करके देते हैं,सैंपल पेपर भी आपको भेज रहा हूँ धन्यवाद,
